Community Builder Award Nomination

Do you know a community partner making bold strides toward inclusion? Help us honor them by submitting a nomination for the ANCOR Foundation’s Community Builder Award.

To be considered, complete the nomination form below by the deadline. Once the window closes, our Awards & Recognition Committee will carefully review all submissions. Finalists may be invited for a short virtual interview, and one honoree will be selected each year.

Community Builder Award winners are celebrated at ANCOR Connect, with the Foundation covering round-trip airfare and one night of accommodations so the honoree can join us in person.

Please note: This award is for community partners, not provider organizations. While Full ANCOR members are not eligible to receive the award themselves, they are encouraged to nominate partners whose work advances our shared mission. Preference will be given to nominations from ANCOR members or to community partners with an explicit ANCOR member connection.

Please provide no more than two declarations of support from a community member noting the worthiness of this initiative for the 2024 Community Builder Award. Declarations of support can be in the form of 1) a letter of recommendation written and signed by the community member endorsing the nominee, and/or 2) a short video testimonial (not to exceed two minutes in duration) from the community member endorsing the nominee. Letters of recommendation should be submitted in PDF format, while video testimonials should be submitted in .mp4 format.
